Comparison Of The Preemptive Effects OF Ibuprofen And Paracetamol In Gynecological Laparotomy

Abstract


Postoperative pain following gynecological laparotomy is a clinical problem requiring optimal analgesic management. Intravenous ibuprofen and paracetamol can be used as components of multimodal analgesia; however, comparative evidence regarding their effects on pain intensity and inflammatory response in patients undergoing gynecological laparotomy remains limited. This study aimed to compare postoperative pain intensity and changes in the Neutrophil-to-Lymphocyte Ratio (NLR) in patients undergoing gynecological laparotomy who received preemptive intravenous ibuprofen or paracetamol. This experimental pre-post test study included 32 patients divided into ibuprofen (n=16) and paracetamol (n=16) groups. Pain intensity was assessed using the Numeric Rating Scale (NRS) before surgery and at 2, 6, 12, and 24 hours postoperatively. NLR was assessed preoperatively and 24 hours postoperatively. Between-group comparisons were performed using statistical tests according to data distribution. No statistically significant differences in NRS scores were found between the groups at the preoperative, 2-, 6-, 12-, or 24-hour assessments (p>0.05). At 6 hours postoperatively, the mean NRS score was lower in the ibuprofen group than in the paracetamol group, but the difference was not statistically significant (2.88±0.62 vs. 3.31±0.79; p=0.102). The mean change in NLR also did not differ significantly between the paracetamol and ibuprofen groups (8.60±16.91 vs. 8.79±15.08; p=0.836). Thus, this study found no statistically significant differences between preemptive intravenous ibuprofen and paracetamol in postoperative pain intensity or changes in NLR following gynecological laparotomy.
