Tumor Krukenberg dengan Metastasis Signet Ring Cell pada Ovarium

Abstract


Tumor krukenberg merupakan tumor primer yang dapat metastasis ke ovarium, berasal dari ekstragenital/ekstra-Mullerian tumor (colon, gaster, traktus biliaris, pankreas, dan appendiks) dan payudara. Seorang perempuan 15 tahun, belum menikah, mengeluh perut kembung dan nyeri perut. Intraoperatif tampak massa padat pada ovarium kiri dan kanan dengan kesan Neoplasma Ovarium Padat (NOP) bilateral curiga ganas dan dilakukan salpingoooforektomi bilateral. Operasi bersama dengan bagian Bedah, tampak tumor sudah menginfiltrasi kolon sigmoid, dan dilakukan transversostomi. Patologi anatomi didapatkan kesan suatu signet ring cell type musinous cyst adenocarcinoma bilateral metastasis ke tuba kiri dan jaringan terpisah tanpa keterangan serta ditemukan sel-sel neoplasma pada bilasan peritoneum dengan DD/ Krukenberg tumor malignant, dan disarankan pemeriksaan imunohistokimia CK7, CK20, dan CD 117 dan konfirmasi MRI terhadap organ sekitar. Didiagnosis sementara dengan ca ovarium inadequate staging dan menjalani neoadjuvan kemoterapi Paclitaxel-Carboplatin. Pemeriksaan imunohistokimia kesimpulan sesuai suatu signet ring cell type (poorly differentiated adenocarcinoma) ovarium bilateral dengan asal dari gastrointestinal (Krukernberg tumor malignant). Simpulan, Tumor Krukenberg merupakan metastasis signet ring cell adenokarsinoma pada ovarium. Primer tumor Krukenberg sering berasal dari adenokarsinoma gaster (pilorus) tipe signet ring cell (70%), namun dapat juga berasal dari kolon, appendiks, dan payudara.
