MANIFESTASI KLINIS DAN LUARAN INFEKSI COVID-19 DENGAN DIABETES MELLITUS

Abstract


Infeksi COVID-19 pada pasien dengan diabetes mellitus dikaitkan dengan manifestasi klinis yang lebih berat dan  luaran yang lebih buruk. Studi ini bertujuan untuk mengetahui manifestasi klinis dan luaran COVID-19 pada pasien diabetes mellitus di Pondok Observasi Gelora Joko Samudro, Gresik. Studi ini merupakan studi kohort terhadap pasien COVID-19 tanpa gejala/bergejala ringan dengan diabetes mellitus di Pondok Observasi Gelora Joko Samudro Gresik Agustus 2020-Maret 2021. Kelompok kontrol merupakan pasien COVID-19 tanpa komorbid dengan kesamaan jenis kelamin dan usia. Didapatkan 19 subjek sebagai kelompok diabetes. Kelompok diabetes mellitus memiliki resiko sekitar 6 kali lebih tinggi untuk mengalami gejala dibandingkan dengan kelompok kontrol (HR 6,04 IK95% 1,34-27,2). Dibandingkan dengan kelompok kontrol, kelompok diabetes juga memiliki angka konversi negatif di akhir perawatan lebih rendah (HR 0,061 IK95% 0,013-0,286), rerata durasi peluruhan kuman sekitar 5 hari lebih lama (Selisih Rerata 5,7 IK95% 3,73-7,75) dan rerata durasi rawat inap sekitar 5 hari lebih lama (Selisih Rerata 5,3 IK95% 3,07-7,46). Keterbatasan dalam jumlah sampel mempengaruhi hasil penelitian ini. Diperlukan studi dengan jumlah yang lebih besar dan mencakup pasien dengan gejala sedang/berat untuk hasil yang lebih baik. Pasien dengan diabetes mellitus memiliki manifestasi klinis dan luaran COVID-19 yang lebih buruk dibandingkan dengan pasien tanpa komorbid diabetes mellitus.
