BADAI ELEKTRIK: DIAGNOSIS DAN MANAJEMEN

Abstract


Badai elektrik (BE) didefinisikan sebagai terjadinya tiga atau lebih episode aritmia ventrikel dalam 24 jam. Penting untuk mengetahui dan mengidentifikasi cara mendiagnosis dan tatalaksana pasien BE karena sering muncul sebagai keadaan darurat medis. BE terjadi terutama pada pasien usia lanjut dan laki-laki. BE dapat muncul dengan berbagai gejala seperti sinkope, gagal jantung, henti jantung, atau terapi ICD multipel (pemacu dan/atau shock anti-takikardia berulang). Beberapa pemeriksaan penunjang seperti EKG, elektrolit dan Transthoracic Echocardiogram (TTE) mampu membantu mengidentifikasi BE dan penyebabnya. Pada BE dengan keadaaan hemodinamik tidak stabil, perlu disiapkan cardiac life support dengan resusitasi jantung-paru yang efektif dan dilakukan defibrilasi emergensi bila diperlukan. Perlu dipertimbangkan untuk dilakukan sedasi dan ventilasi pada pasien yang sulit ditangani. Selain itu terapi farmakologis seperti beta-bloker, amiodaron dan sotalol dapat membantu menghentikan aritmia ventrikel. Terapi definitif BE adalah ablasi kateter ,pilihan terapi lain yang tersedia adalah modulasi sistem autonomik, dan Stereotactic Body Radiation Therapy (SBRT) dan dapat digunakan pada BE refrakter. Banyak penelitian telah menunjukkan bahwa pasien dengan BE memiliki prognosis yang lebih buruk dan tingkat kematian yang lebih tinggi daripada mereka yang tidak mengalami BE sehingga tinjauan pustaka ini bertujuan untuk membahas diagnosis, penanganan dan prognosis dari BE.
